Before we can do a Teledermatologist consultation, there’s some paperwork that needs to be filled in.
As a practice manager, receptionist, nurse or administration officer, please provide patients with a copy of the following document.
Ideally the best time to provide them with a copy of the form is as soon as they inform you that they need to be referred to a specialist. As this provides them with the maximum amount of time to read through, process & fill out the form.
